Dear Parents,

 Each year we need to up-date all Medical Release Forms for anyone under the age of eighteen.  The form has changed slightly this year.  Please notice the date of validity under the heading.  This form will be valid from June 1, 2005 through May 31, 2006. This will also help us keep all forms current. If the information changes during the year, please submit an updated form.

 We appreciate your help in keeping this information up-to-date.  Our hope is that it will not be needed, but it is necessary to have in hand should an accident or illness occur while on an outing with First Baptist Church.

 The Photo Consent information is a legality due to privacy laws and the use of activity pictures in church publications and on the website even though we do not publish names with the pictures.

 Also, please remember that while we welcome guests of your children or youth at most of our events, they too must have a notarized form on file.  Additional forms may be secured from any of the Ministry Assistants or downloaded from our website. All forms should be completed and registered in the church office at least 24 hours before an event.
